DONATION WAIVER AND RELEASE OF LIABILITY
I, donee of the masks, by accepting the masks, assume all of the risk of accepting the donated masks that may arise from defects of the products, or negligence of the persons or entities involved in the donation process. I waive, release and discharge MasksForAllSF, Communities as One, SFCDMA (Masks Donors) and their volunteers, affiliates and participants all the liabilities. I indemnify, hold harmless and promise not to sue Masks Donors and its volunteers for any and all liabilities or claims made as a result of accepting the donated masks, whether caused by the negligence of donation, or product defects or otherwise. BY ACCEPTING THE DONATED MASKS, I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT.
